When patients move between one health care facility and another, or between a facility and their homes, careful transitional care is necessary. In fact, it is common for a patient’s care needs to change during the course of treatment for acute or chronic disorders.
For our older clients, we often need to coordinate various treatment regimens at different facilities due to the complicated nature of certain illnesses. Younger patients with serious conditions typically need to be transitioned to adult care facilities after they reach maturity.
Especially important in cases where complex needs are present, transitional care is a vital part of a patient’s complete treatment plan. Numerous factors must be taken into consideration, like the patient’s clinical status, goals, preferences and self-confidence. Part of the transitional care plan usually includes educating family members about a patient’s status and prognosis.
