The period immediately following hospitalization is often one of the most important times in a patient’s recovery.

Hospital follow-up visits allow us to review the reason for hospitalization, reconcile medications, discuss new diagnoses, review laboratory and imaging results, monitor recovery, and coordinate additional care recommended by specialists.

Careful follow-up reduces confusion, helps prevent medication errors, identifies complications early, and may reduce the risk of hospital readmission.

We encourage recently hospitalized patients to schedule follow-up visits promptly after discharge.
